Contact Us
Appointments
To request an appointment, call our office at 920-749-4000 or fill out the form below. A patient service representative will contact you to schedule your visit.
Prescriptions
Please call our office at 920-749-4000 directly and have the phone number of your pharmacy available when you call.
Some narcotic (pain) prescriptions are not refillable by phone. In that case, you would need to come to our office to pick up the written prescription. Be sure to call first so that we can have it ready for you.
Please allow 48 hours for us to process your prescription renewal request.
Billing Questions
We have addressed many common financial questions on our financial help desk page. You may also call our billing office at (920) 749-4000.
Test Result Questions
During your appointment, your nurse will explain how your test results will be communicated to you. Many times the doctor will review the results and then have the nurse call you with results and explain to you if further testing or treatment needs to be done.
We try to notify you of laboratory results as soon as they are available. If you do not hear from us, we encourage you to call us during regular office hours so that your medical record and results are available to the physician.
We do send you a letter when your pap smear comes back within normal limits. If you do not hear from us within 3 weeks of your appointment, please call our office.
Requesting Medical Records
To request your medical records please download WHS Medical Records Release Form, complete it, and return it to our office. We will provide your records to another healthcare provider as a professional courtesy at no charge. If you need a copy for yourself we do charge a nominal fee of $0.50 per page.
I have a general question

Due to our privacy practice, we will not respond to online/email inquiries that pertain to medical advice or patient-sensitive information. If you have a medical question, please call our office during business hours and ask to speak to your physician’s nurse.
